OnBoard Surveys allow administrators to collect structured feedback from board or committee members. Survey responses are automatically recorded and can be exported for reporting, compliance, or record-keeping.

Creating a New Survey
How Do You Create a New Survey in OnBoard?
To create a new survey in OnBoard, follow these steps:
- In the left navigation, click Actions
- Select Create New
- Choose Basic Survey
- Click Create Survey
How Do You Configure Survey Details?
After creating the survey shell, you can enter the survey details:
- Enter a Survey Name
- Optionally set:
- Open Date – Automatically sets the Survey to Open.
- Closed Date – Automatically sets the Survey to Closed.
- Add a Description
How Do You Set Survey Notifications?
To control email notifications:
- Click Set Open & Close Notifications
- Select whether users are notified when:
- The Survey opens (On Survey Open)
- The Survey closes (On Survey Close)
| NOTE: Automatic notifications are only sent when the Survey is set to auto open/close and not when manually opening or closing. Additionally, Survey administrators do not receive these notifications. |
How Do You Add Supporting Documents?
You can attach documents to the overall survey:
- Upload files such as PDF, Word, Excel, or PowerPoint
- These provide additional context for participants
Always save your progress before continuing:
- Surveys begin in Draft status
- You can edit the survey until it is opened
Adding Respondents to a Survey
How Do You Add Users to a Survey?
To add participants:
- Select Manage in the Permissions panel
- Add users by:
- Groups (recommended)
- Individual members
Why Should You Use Groups For Respondents?
Using groups is recommended because:
- Ensures no users are missed
- Automatically includes new members added to the group
How Do You Assign Group Respondents?
- Go to the Groups tab
- Click the + icon next to the desired group(s)
How Do You Add Individual Respondents?
- Open the Member tab
- Click the + icon next to each user

Permission Types
What Survey Permission Types are Available?
Understanding how permissions work in Onboard is critical to allowing you and your organization to access and edit your Surveys. Below is the list of different permissions types for the Surveys action.
| Respondent | Respondents can see the Survey details and respond to Survey questions. |
| Admin | Survey Admins have full control of the Survey. Admins can update Survey details, permissions, settings, questions, and statuses. Survey Admins do not have permission to respond to Survey questions. |
| Admin and Respondent | Users with this permission have full Admin permissions, and can also respond to Survey questions as a Respondent. |
| Exclude | Only used when assigning permissions through Groups. Completely remove a user from viewing, accessing, and being notified of this survey. |
Permission Behavior to Remember
- Individual permissions override group permissions.
- Respondents can receive an email notification after the Survey is marked Open (if notifications are set).
- You can add Respondents any time before the Survey closes, but Respondents added after it opens will not get the original email notification and should be notified directly.
- Respondents who are removed from the Survey after it has opened will still appear in the survey results.

When done, select Save Changes to save the permissions.
Creating Survey Questions
How Do You Add Survey Questions?
To begin building your survey:
- Click Compose Survey
- Select Add Question
What Types of Survey Questions are Available?
You can choose from a few different Question Types by using the dropdown menu. Some of the question types will have a default set of answers that will pre-populate for you.
- Net Promoter Score – This is an index ranging from zero to ten that measures the willingness to recommend a company’s product or service to others. When selecting this type of question, the zero through ten answer choices will be populated for you automatically, and they cannot be edited.
- Check Boxes – This type of question will be used when you wish to display multiple different answer choices, and allow the survey participant to be able to select more than one
- Radio Buttons – This type of question will be used when you wish to display multiple different answer choices, and allow the survey participant to be able to select only one.
- Likert Scale – Likert Scales are used to gauge the participant’s attitude or opinion of the question. With questions of this type, participants will need to indicate the extent to which they agree or disagree. Selecting this option will populate a few answer options automatically, but they can be edited to your liking.
- Essay / Long Answer – This type will elicit a more verbose and detailed response from the survey participant. When using this question type, participants will need to key in their own text as an answer.

How Do You Configure Question Settings?
After selecting a type:
- Enter your question text
- Enable Require This Question if mandatory
How Do You Create Response Options?
To define answer choices:
- Click Add Response
- Enter response text
- Repeat as needed

What is the "Allow Write-In" Option?
The Allow Write-In option lets users enter custom responses.
Note:
- A write-in response replaces selected answers
- For both selection + text, add a separate Essay question instead
How Do You Attach Documents to Questions?
- Drag and drop files OR click Add Document
- Supported formats: PDF, Word, Excel, PowerPoint
How Do You Control Survey Result Visibility?
When creating each question for your survey, you have a few different options for displaying results. First, choose who can see results:
- Results Visible to Admins: Results are visible to Admins only
- Results Visible to Respondents: Results are visible to both Respondents and Admins
Then select one of the required display types:
- Graphical: Anonymous results that show the percentage of each selection made for the question.
- Roll Call: Displays the name of each respondent and their responses for the question.
| NOTE: If "Graphical" and/or "Roll Call" results are deselected, Survey Admins will still have access to all results of the Survey in the downloaded results. |
How Do You Reorder or Delete Questions?
On the Compose Survey screen, select any of your questions in the list. Then use:
- Move Up
- Move Down
- Delete
How Do You Edit Survey Questions?
Select the Edit Details button on a question to edit it.

Questions can only be edited when the Survey is in Draft status. Once opened, the Survey and questions cannot be edited. Individual Survey questions cannot be copied or duplicated. |
Opening a Survey
How Do You Open a Survey For Responses?
To make the survey available:
- Select Open from the Status dropdown
- Confirm your selection by selecting Open from the pop-up.
When the notification pop-up appears:
- Optionally send notifications
- Add a custom message

Can Surveys Open Automatically?
Yes. If an Open Date is set, the survey will:
- Open automatically
- Notify respondents
- Begin collecting responses
Responding as an Administrator
Can Admins Respond to Surveys?
Yes, if they also have Respondent permissions.
- After opening, click Take Survey.
Sending Survey Reminders
How Do You Remind Users to Complete Surveys?
To send reminders:
- Click Send Reminder
- Select either:
- Send to all participants
- Send to those who have not yet submitted a response
- Add a custom message
Closing a Survey
How Do You Close a Survey?
To close a survey:
- Select Close from the Status dropdown
- Confirm the action by selecting Close Survey (cannot be undone)
- Once Close Survey is selected, you will be presented with the option to notify participants that the Survey has closed and include a custom message.
What Happens When a Survey Closes?
- Survey becomes locked
- No more responses allowed
Alternatively, the survey will auto-close when the End Date is reached
Viewing and Exporting Survey Results
How Do You View Survey Results?
Once the Survey is closed, you can review each question’s results by selecting View Results.
How Do You Export Survey Results?
- Click Download Results
- Choose format:
- Microsoft Word
- Microsoft Excel

Deleting or Duplicating a Survey
How Do You Duplicate a Survey?
To reuse a survey:
- Click the Three-dot options menu next to the survey
- Select Duplicate
How Do You Delete or Share a Survey?
From the same menu, you can:
- Delete the survey
- View it
- Copy the survey link

FAQs
Can I edit a survey after it has been opened?
- No. Surveys and questions cannot be edited once the survey is opened.
Do survey admins receive notifications?
- No. Survey Admins do not receive open or close notifications.
What happens if I add respondents after opening the survey?
- They will not receive automatic notifications and must be contacted manually.
Can I reuse a survey?
- Yes. You can duplicate an existing survey instead of recreating it.
What is the difference between Graphical and Roll Call results?
- Graphical: Anonymous percentage-based results
- Roll Call: Shows individual responses with names
Can a question have both selected answers and written input?
- No. Write-in responses override selected answers. Use a separate Essay question if needed.
Is there a limit to responses or options?
- Yes:
- Max 11 responses per question
- Max 11 answer choices
Can survey results be downloaded before the survey is closed?
- No, survey results cannot be downloaded until the survey is closed. The download option becomes available only after the survey has been fully closed, either manually or automatically based on the End Date.
