How can I create an esignature document that also provides space for members to type in additional information? I'm needing our board members to sign off on our conflict of interest policy form, but need to allow them to type in any applicable conflicts they may have.

Rachel, this is possible with our Director & Officer Questionnaires feature. You can review our help article on D&O Questionnaires here.
